Avanafil is a prescription-only medicine in the EU/EEA used to treat erectile dysfunction in adult men. It belongs to the class of medicines known as phosphodiesterase type 5 (PDE5) inhibitors and is the active substance of Spedra, the avanafil medicine authorised across the EU through the European Medicines Agency. Sexual stimulation is needed for it to work.
How it works
Avanafil is a highly selective, reversible inhibitor of the PDE5 enzyme. When sexual stimulation releases nitric oxide in the penis, blocking PDE5 lets cyclic guanosine monophosphate (cGMP) build up, which relaxes the smooth muscle and lets blood flow into the penile tissue to produce an erection. The EU product information states that avanafil has no effect without sexual stimulation. It is absorbed quickly: peak blood levels are reached roughly 30 to 45 minutes after a dose taken on an empty stomach.
How to take it
Avanafil is taken by mouth, as needed, approximately 15 to 30 minutes before sexual activity, at the dose your doctor prescribes. It must not be taken more than once a day. - Not to be used by men who have had a heart attack, stroke or life-threatening heart-rhythm problem in the last 6 months, or who have unstable angina, angina during sex, or moderate-to-severe heart failure.
- Not to be used with very low (below 90/50 mmHg) or very high (above 170/100 mmHg) blood pressure, severe liver or severe kidney impairment, or after loss of vision in one eye from NAION.
- Must not be combined with potent CYP3A4-inhibiting medicines such as ketoconazole, itraconazole, ritonavir or clarithromycin.
- Alpha-blockers, other blood-pressure medicines and alcohol can each add to the blood-pressure-lowering effect and cause dizziness or fainting.
- Avoid grapefruit juice within 24 hours before taking avanafil.
- Do not combine with other PDE5 inhibitors or other erectile-dysfunction treatments — the combinations have not been studied.
- Not indicated for women or for anyone under 18.
