What is Sildenafil?

Sildenafil is the active substance of Viagra and a prescription-only medicine in the EU/EEA, used for erectile dysfunction in adult men: difficulty getting or keeping an erection firm enough for sex. It is a phosphodiesterase type 5 (PDE5) inhibitor, like tadalafil, vardenafil and avanafil. The EU product information sets 100 mg as the maximum dose, at most once a day, and it only works with sexual stimulation.

How it works

Sildenafil works by blocking the PDE5 enzyme, which allows levels of a substance called cyclic guanosine monophosphate (cGMP) to rise in the tissues of the penis. This helps relax the blood vessels and improve blood flow when a man is sexually stimulated. Sexual arousal is still needed for the medicine to take effect; it does not cause an erection on its own. The EU product information says to take it about 1 hour before sexual activity; in a clinical study it still produced erections 4 to 5 hours after a dose.

How to take it

Sildenafil is taken by mouth about one hour before sexual activity, at the dose your doctor prescribes. Caution is needed if you have low blood pressure or take alpha-blockers or other blood-pressure-lowering medicines.
