Sildenafil Drug Interactions: Medicines and Substances to Watch
Reviewed by the Kam4eu Pharmacy Team on 14 March 2026 · Next review June 2027
Sildenafil is one of the most widely used treatments for erectile dysfunction (ED), but like any medicine it can interact with others. Some interactions are merely worth monitoring; others are genuinely dangerous. Knowing which is which — and always sharing a full medicines list with your doctor — keeps treatment safe.
How sildenafil can interact
Sildenafil is a PDE5 inhibitor that lowers blood pressure slightly as part of how it relaxes blood vessels. Most significant interactions stem from this blood-pressure effect or from other medicines changing how much sildenafil ends up in your system. See our Sildenafil guide for the basics.
The most important interaction: nitrates
Sildenafil must never be taken with nitrate medicines. Nitrates are used for chest pain (angina) and some heart conditions, and recreational "poppers" (amyl/butyl nitrites) belong to the same group. Combining them with sildenafil can cause a sudden, severe and potentially life-threatening drop in blood pressure. There is no safe gap or workaround — the two must not be mixed. We cover this in depth in our guide to ED medication and nitrates.
Alpha-blockers and blood-pressure medicines
Alpha-blockers (often used for prostate symptoms or high blood pressure) can add to sildenafil's blood-pressure-lowering effect, sometimes causing dizziness or fainting. This combination is not always forbidden, but it needs careful medical supervision and dose adjustment. See ED medication and alpha-blockers.
Medicines that change sildenafil levels
Some drugs slow down how the body breaks sildenafil down, raising its level and the chance of side effects:
- Certain antifungal medicines (such as ketoconazole).
- Some antibiotics (such as erythromycin).
- Certain HIV protease inhibitors (such as ritonavir).
Others can reduce its effect. Your doctor may adjust your dose or timing if you take any of these.
Food and drink
- Grapefruit and grapefruit juice can interfere with how sildenafil is processed. See our dedicated guide to grapefruit and ED medication.
- A large, fatty meal can slow the onset of sildenafil.
- Heavy alcohol can worsen side effects such as dizziness and may make erections harder to achieve.
Other PDE5 inhibitors
Never take sildenafil alongside another ED tablet such as tadalafil or vardenafil. Doubling up does not improve results and increases the risk of side effects.
How to stay safe
- Give your doctor and pharmacist a complete list of prescription medicines, over-the-counter products and supplements.
- Mention any recreational substances honestly — especially poppers.
- Read the patient information leaflet for the interactions section.
- Seek urgent care for an erection lasting over four hours, chest pain, fainting, or sudden vision or hearing changes.
Summary
Sildenafil is generally well tolerated, but its interactions matter. The absolute rule is no nitrates or poppers; alpha-blockers and certain other drugs need caution and supervision. When in doubt, ask a pharmacist before combining anything. Explore the erectile dysfunction range to learn more.

Frequently asked questions
What should you not take with sildenafil?+
You should never take sildenafil with nitrate medicines or recreational poppers, as the combination can cause a dangerous drop in blood pressure. Caution is also needed with alpha-blockers and certain antifungals, antibiotics and HIV medicines.
Can I drink alcohol with sildenafil?+
A small amount of alcohol is usually tolerated, but heavy drinking can worsen side effects like dizziness and make erections harder to achieve. It is best to keep alcohol moderate and check the leaflet.
Does sildenafil interact with blood-pressure medication?+
It can add to the blood-pressure-lowering effect of alpha-blockers and some other drugs, sometimes causing dizziness. This often requires dose adjustment and medical supervision rather than being strictly forbidden.
Can I take sildenafil with another ED tablet?+
No. You should never combine sildenafil with tadalafil, vardenafil or any other PDE5 inhibitor, as this raises the risk of side effects without improving results.
Does grapefruit affect sildenafil?+
Yes, grapefruit and grapefruit juice can interfere with how sildenafil is broken down, potentially increasing its level in the body. It is sensible to avoid grapefruit around the time you take it.
